Next Steps for Enrollment for 2012-2013 School Term

Now that you have been accepted to St. Croix High School, here are a few reminders about your next steps.

PRE-REGISTERING FOR CLASSES

In order to pre-register you for classes and provide you with the best classes for your needs, you must provide St. Croix’s Registrar and International Student Advisor with the items listed below before May 1, 2012.

• TOEFL TEST SCORE
Submit a TOEFL score by May 1, 2012 for English placement. Without a TOEFL score, a student will be placed in ESL I (English Second Language Level I.) If a student cannot take a TOEFL prior to arrival in August, they will remain in ESL I until they take the TOEFL test. St. Croix’s TOEFL number is 1896.

• MATH PLACEMENT TEST
Take the math placement test included in this packet. Mail or fax the test to St. Croix by May 1, 2012. Instructions for taking the math placement test are on the first page of the test.

• CURRENT CLASS SCHEDULE
Send or fax a translated copy of the classes that you are taking at your current school. The final report card from these classes must be included with the student’s Official Transcripts that new students must bring with them in August. (See “OFFICIAL TRANSCRIPTS” below.) This is due by May 1, 2012.

• INTERNATIONAL STUDENT COURSE REGISTRATION: ELECTIVE PREFERENCE SHEET
Complete the form enclosed in this package and fax or mail it to St. Croix. This sheet tells us what courses you are interested in taking and early registration in to these classes helps to reserve your place. This is due by May 1, 2012.

Place these items in the white envelope with the St.Croix address stamped on it and send it to us by May 1, 2012.


OBTAINING YOUR VISA

• Apply for your F-1 visa as soon as possible. You must pay a US$200 SEVIS I-901 fee. The fee can be paid by mail using a Form I-901 together with a check or money order drawn on a US bank and payable in US dollars OR be paid electronically by completing Form I-901 through the internet using a credit card. For complete information on how to pay and who needs to pay this fee, please see the SEVIS website: www.ice.gov/sevis/ and click on “International Students.”

• When a visa is granted, please inform St. Croix HS immediately.


CHECKLIST OF REQUIRED FORMS

Below is a list of the forms that you need to fill out carefully and send to St. Croix by August 1 or bring with you when you come. Please read the information below explaining how to fill out the forms.

• OFFICIAL TRANSCRIPTS

When you arrive at St. Croix, bring your OFFICIAL final transcript of your grades at the conclusion of your current academic year. An official transcript is one with a school official’s signature, chop, or the school seal. Faxes are not accepted. Photocopies are not accepted unless they bear the official witness of the school, including a school official’s signature, chop, or the school seal. We MUST have your final transcript to register you for classes.

• PUPIL IMMUNIZATION RECORD (white paper)

Parent or physician fills out the immunization record writing the month, date, and year that the child received each vaccination. A parent must sign and date the form. Also provide the information required as to whether the child has had chicken pox and date and sign that section.

• MSHSL SPORTS QUALIFYING PHYSICAL EXAMINATION CLEARANCE FORM (orange paper, 3 pages)

Minnesota State High School League Sports Qualifying Physical Examination Clearance Form is required. This must be completed in English, signed and stamped by the physician. If you want to play sports, you are required by MN state law to have a physical examination by a physician. If you do not have this, you cannot participate in any extra-curricular sports. Be sure to include parent signatures also (English or native language).

• MSHSL ATHLETIC ELIGIBILITY STATEMENT WITH HEALTH QUESTIONNAIRE (yellow, 1 page front and back)

Please complete form. Parents sign on both sides.

• AUTHORIZATION FOR ADMINISTRATION OF MEDICATIONS (purple paper)

This form allows dormitory teachers and homestay parents to administer over the counter medications as well as prescribed medications to students if necessary. Please mark any medications that you should not be given. A parent signs this form.

• EMERGENCY AND MEDICAL CONSENT FORM (yellow, 1 page)
Please complete. A parent signs this form.

• PHYSICIAN’S ORDER FOR ADMINISTRATION OF MEDICATIONS (purple)
A parent needs to complete and sign this form if your son or daughter has been prescribed medications. This includes any antibiotics prescribed from a dermatologist as well as any prescriptions for an on-going condition such as asthma.

• FIELD TRIP AND OFF-CAMPUS ACTIVITIES PERMISSION FORM (purple)
A parent must complete and sign this form.

• i.s.m. ON-LINE APPLICATION DIRECTIONS

Medical/accident insurance for international students attending school in the USA is required to activate your student’s I20. Included are directions for applying for insurance on-line. Print out a copy of the enrollment card and bring this with you when you arrive at St. Croix.

Coverage must begin before the student arrives at St. Croix. If the student has other accident/medical
insurance that provides coverage while in the United States, we will need a copy of the student’s insurance card as proof. Please send a copy by August 1. If your insurance company is not based in the USA, you may be required to pay all medical visits up front and then be reimbursed from your insurance company. All students must have coverage through June 10, 2013.

PAYMENT

• ENROLLMENT FEE
After receiving your letter of acceptance, send the $2,000 enrollment fee to guarantee your enrollment. The invoice for this payment is included in this acceptance packet with the wire transfer information on the invoice.

• TUITION AND RESIDENCE FEE
Final payment of $22,200 is due no later than August 1, 2012. Payment is to be made by a bank draft or by wire transfer. (Wire transfer information enclosed.) The invoice for this payment is included in this acceptance packet.


IMPORTANT TRAVEL AND ARRIVAL DATES AND INFORMATION

• A school calendar for the 2012-2013 academic year is enclosed. Please note important dates for arrival and Christmas and Spring breaks. It is important that flight arrangements are made so that students do not miss any school days.

• All international students must enter the United States with financial information showing proof of necessary funds to cover tuition and fees. Please obtain this proof from your bank or other financial institution and attach it to the student’s I-20 form. The student should carry this with him/her whenever he/she travels.

• New international students should arrive on Monday, August 20, or Tuesday, August 21. The school provides pick-up for students on these two days. If you arrive in the USA earlier than these days, you must arrange your own airport pick-up and accommodations.

• Please notify us as soon as you know your flight time on Monday 8/20 or Tuesday 8/21. (If you do not need us to transport you from the airport, please let us know this information.) International Student Advisor email : sgaller@stcroixschools.org

All new international students are required to be present for class registration and orientation on Wednesday, August 22 through Friday, August 24. Classes for the 2012-2013 school year begin on Monday, August 27, 2012.


STUDY ENGLISH!

St. Croix provides a full ESL program for our International students. While it is not required, those who are interested in doing all they can to prepare themselves in English may enroll in the spring/ summer English programs below at an extra cost. These extra English classes are suggested for any students who have not had multiple years of English or who have a score of 45 or less on the SLEP test.

ST. CROIX SUMMER ENGLISH LANGUAGE EXPERIENCE

Enclosed is a brochure for St. Croix’s Summer English Language Experience. You will find all the information on dates and fees in the brochure. The Summer English Language Experience is an enjoyable summer program that St. Croix teachers and students operate. After the Summer English Language Experience you may register with Global Language Institute (GLI) (see below) for an additional English program. Arrangements will be made between St. Croix and GLI to provide transportation back to St. Croix after the GLI program is complete.

GLOBAL LANGUAGE INSTITUTE
Global Language Institute is a school near St. Croix High School that offers an Intensive English Program (see brochure or www.gli.org ) in four-week sessions. GLI is offering a 3-week English program especially for St. Croix students. This program is offered from August 1 through August 19, 2012. The price for GLI’s 2012 three-week program is $1,963 plus a $100 application fee. This includes tuition, homestay (room and all food), homestay placement fee, pickup and drop-offs, health insurance, materials, activities and entrance fees, facility and technology fees and bus transportation.

St. Croix does not accept payment or registration for the Global Language Institute. For further information, registration, and payment of fees contact: MaryGuion@gli.org.

St. Croix Summer English Camp students will be picked up at the St. Croix dormitory by the homestay arranged by GLI on the evening of Sunday, July 31. This homestay family will take the students back to the St. Croix dorm on the evening of Sunday, August 19.
