Students participating in Minnesota State High School League (MSHSL) sponsored sports or activities are required to submit an eligibility form. This form must be on file in the school office before the sports season begins.

A copy of the eligibility brochure and form is available here for download. Parents and students should read through the brochure provided, then complete it, sign the form on front and back, and return the form to the school office.  New this year is information on concussion management for all coaches, staff, parents and students.
